Yes your main KW needs to be in the important places such as URLs and Titles, but it is not as important as it used to be, especially not in the way that tools or plugins would suggest, because where they would say your SEO is good might actually be over optimized because you mentioned the same phrase too often. So be careful of following the outdated tools.

There are many (many, many) factors that go in to ranking, so just focusing on one as the winning strategy isn't going to work. Plus including too many keywords can hurt your site. Better to focus on creating content that adds value for your site visitors, and as others have mentioned, be sure to include secondary keywords and related terms.  :) Good luck!

Yes, keyword density is still important for SEO. Because some of the high DA website allow keyword density function. Such as Articlebase, Sooperarticles which have high DA and allow only a fix amount of same keywords. We cannot excess the quantity of that word.

Actually. The answer is a resounding NO!
Google now looks at content. It has to flow naturally. Matt Cutts, had a talk about that exact same thing.
He recommends that using your keyword 3 to 4 times is more than enough. Why?
Because in your content it should become clear what your page is about. Stuffing it with keywords now can get you labeled as a spammy site. Thus your site should not be clear based on repeating a keyword 8 times. It should be clear by the text you surround on topic to that keyword.

Think less about density and more about developing your theme.

Have your target phrase(s) in mind then make sure to hit the following locations with it or a highly related plural or synonym.

Title, headline, URL, body copy (italics, bold) and maybe even in outbound anchor text or some inter linking.

Get at it as often as its naturally applicable. Focus on fully covering the idea/theme you've set out to share, in no less than 300 words.
