URL optimization.. Help needed

I have optimized my URL as per google standards, but again when I search with keyword, the google is showing the previous URL.. Can anyone help me on this.. Do I need to do some changes in webmaster or what should I do get my cleaned URL appear when I type in google using the Keyword...

1. make 301 redirect in .htaccess file from old URL to new
2. it takes some time for google to reindex your page address (up to month)

I just wanted to know in .htaccess what we need to do exactly.. Can u please reply me in brief so that I can start implementing on it...

Make redirects like this:
Redirect /old_dir/page http://www.yourdomain.com/new_dir/page
/old_dir/ is your old directory without domain name
