My Keyword Rankings are continuously getting down

Can anyone suggest me why my keywords are getting down. From starting of this month I saw the ranking drop. What shall I do now??? I check with Google algorithm the last core algorithm may affect. Is that true?? Give a idea to resolve it.

If your doing continuous changes in your website, then it might cause to drop keyword ranking.
