How many times should you repeat a keyword in say every 100 words? I heard repeating a keyword too many times can actually have a bad affect (spamming).

But for Keyword Density Tania is right, keyword Density must be 3% to 4% not more than that, It means that if you are writing 100 words, You can write your targeted keyword 3 times or 4 times.
If you are using it more than it, You might be counted as a Spammer.

There is not any specific formula calculate the percentage of your keyword. It must be 3 to4 %, Like in a content of 400 words your keyword should not exceed from 6to 7 times.
According to me 2 to 3% keyword density can be uses, if it is more then it counts as keyword stuffing. google penguin algorithm is against keyword stuffing you should be careful with that.
