Why aren't all my pages being indexed?

I saw many times that Google indexed my some pages not all pages. How can i get my all pages index from Google? Please suggest your point of view.

A few reasons for this,
- Google is not indexing your pages with duplicate content
- Google is unable to discover those pages
- These pages are not mentioned in the Sitemap

make sitemap.xml and upload to google webmaster tools and make robots.txt
